For example information about an individuals:<\/p>\n\n\n\n We may process this information for the purposes of medical diagnosis, provision of health treatment and management of health of our patients and the community we serve.<\/p>\n\n\n\n Your GP electronic patient record is held securely and confidentially on an electronic system managed by your registered GP practice. If you require attention from a local health or care professional outside of your usual practice services, such as in an Evening and Weekend GP Out Of Hours, Emergency Department, Minor Injury Unit or Out Of Hours service, the professionals treating you are better able to give you safe and effective care if some of the information from your GP record is available to them.<\/p>\n\n\n\n Where available, this information can be shared electronically with other local healthcare providers via a secure system designed for this purpose. Depending on the service you are using and your health needs, this may involve the healthcare professional accessing a secure system that enables them to view parts of your GP electronic patient record ,or a secure system that enables them to view your full GP electronic patient record (e.g. EMIS remote consulting system).<\/p>\n\n\n\n In all cases, your information is only accessed and used by authorised staff who are involved in providing or supporting your direct care. Your permission will be asked before the information is accessed, other than in exceptional circumstances (e.g. emergencies) if the healthcare professional is unable to ask you and this is deemed to be in your best interests (which will then be logged).<\/p>\n\n\n\n All staff in the Practice sign a confidentiality agreement that explicitly makes clear their duties in relation to personal health information and the consequences of breaching that duty.<\/p>\n\n\n\n Please be aware that your information will be accessed by non-clinical Practice staff in order to perform tasks enabling the functioning of the Practice. These are, but not limited to:<\/p>\n\n\n\n We process prescriptions requests on a daily basis. This involves our staff accessing your information held on our computer database, to produce your prescription(s) that you, your carer, nominated person, or pharmacy has requested. Prescriptions can be requested in various methods, by telephone, online via our computer system, by post, in writing, via fax (if available) or in person. Please refer to our prescription procedure for further information. If using the postal service always allow extra time so you do not run out of your medication.<\/p>\n\n\n\n We\nwill always ask for your details when a prescription uplift request is received\nthis could be your name, date of birth and address, medication you requested,\nor Community Health Index Number (which uniquely identifies you). This ensures\nthat we can produce your prescription(s) efficiently, and reduces the risk of\nan incorrect prescription being given to you or your nominated person\/carer\/pharmacy.<\/p>\n\n\n\n Please\nallow the allotted time before uplifting your prescription. Prescriptions can\nbe collected\/sent via the following methods:<\/p>\n\n\n\n Risk prediction data tools are increasingly being used in the NHS to help determine a person\u2019s risk of suffering a particular condition, preventing an unplanned or (re)admission and identifying a need for preventive information.<\/p>\n\n\n\n Information about you is collected from a number of sources in NHS Scotland including this GP Practice. A risk score is then arrived at through an analysis of your<\/p>\n\n\n\n De-identifiable information by ISD Scotland and is only provided back to your GP\u2019s Data Controller in an identifiable form. Risk prediction enables your GP to focus on preventing ill health and not just the treatment of sickness. If necessary, your GP may be able to offer you additional services.<\/p>\n\n\n\n Patient Privacy Notice Data extracts previously captured via SPIRE are now scheduled to be undertaken by the new PHS Primary Care Intelligence Service (PCIS). The permissions associated with the new service are the same as those previously agreed for SPIRE.<\/p>\n\n\n\n Emergency care information such as your name, date of birth, the name of your GP, any medicines which your GP has prescribed, any medicines you are allergic to or react badly to, is shared with Out of Hours as this might be important if you need urgent medical care when the GP surgery is closed.<\/p>\n\n\n\n NHS staff (doctors, nurses, Accident and Emergency department, ambulance control and crews) can look at your ECS if they need to treat you when the surgery is closed. They will ask for your consent before they look at your records.<\/p>\n\n\n\n In an emergency and if you are unconscious, staff may look at your ECS without your agreement to let them give you the best possible care.<\/p>\n\n\n\n Whenever NHS staff looks at your ECS, a record will be kept so we can always check who has looked at your information. If you wish to opt out of ECS please complete the online opt-out form.<\/a><\/p>\n\n\n\n Key information summary (KIS) has been designed to support patients who have complex care needs or long term conditions.<\/p>\n\n\n\n KIS allows important information to be shared with health care professionals in unscheduled care in the NHS 24, A&E, Scottish Ambulance Service, Out of Hours, hospital and pharmacy environments.<\/p>\n\n\n\n Information contained in KIS summary includes, future care plans, medications, allergies, diagnosis, your wishes, carer and next of kin details.<\/p>\n\n\n\n You have the right to say that you do not want Care staff to see ECS\/KIS. Please contact the Practice.<\/p>\n\n\n\n Health care professionals who provide you with care maintain records about your health and any treatment or care you have received previously (e.g. NHS Trust, GP Surgery, Walk-in clinic, etc.). These records are used to help to provide you with the best possible healthcare.<\/p>\n\n\n\n NHS health care records may be electronic, on paper or a mixture of both, and we use a combination of working practices and technology to ensure that your information is kept confidential and secure. Records this we hold about you may include the following information;<\/p>\n\n\n\n To ensure you receive the best possible care, your records are used to facilitate the care you receive. Information held about you may be used to help protect the health of the public and to help us manage the NHS. Information may be used within the GP Practice for clinical audit to monitor the quality of the service provided. Some of this information will be held centrally and used for statistical purposes. Where we do this, we take strict measures to ensure that individual patients cannot be identified.<\/p>\n\n\n\n Sometimes your information may be requested to be used for research purposes \u2013 the surgery will always gain your consent before releasing the information for this purpose.<\/p>\n\n\n\n The Practice may conduct Medicines Management Reviews of medications prescribed to its patients. This service performs a review of prescribed medications to ensure patients receive the most appropriate, up to date and cost effective treatments. This service is provided to practices within our local GP Cluster.<\/p>\n\n\n\n National Registries (such as the Learning Disabilities Register) have statutory permission under Section 251 of the NHS Act 2006, to collect and hold service user identifiable information without the need to seek informed consent from each individual service user.<\/p>\n\n\n\n If you send us information in connection with a job application, we may keep it for up to three years in case we decide to contact you at a later date.<\/p>\n\n\n\n If we employ you, we collect information about you and your work from time to time throughout the period of your employment. This information will be used only for purposes directly relevant to your employment. After your employment has ended, we will keep your file for six years before destroying or deleting it.<\/p>\n\n\n\n When you contact us, whether by telephone, through our website or by e-mail, we collect the data you have given to us in order to reply with the information you need. We record your request and our reply in order to increase the efficiency of our Practice.
